Terms of Service

Effective date: February 17, 2026

1. Acceptance of Terms

By accessing or using FormLab (“the Service”, “we”, “our”, or “us”), you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use the Service.

These terms apply to all users of the Service, including athletes, coaches, and visitors. Your continued use of the Service after any changes to these terms constitutes your acceptance of those changes.

2. Account Terms

To use FormLab, you must create an account. When creating your account, you agree to the following:

  • You may only maintain one account per person. Multiple accounts for the same individual are not permitted.
  • You must provide accurate, complete, and current information when registering.
  • You must be at least 13 years of age to use the Service. Users under 18 must have parental consent.
  • You are responsible for maintaining the security of your account credentials and for all activity that occurs under your account.
  • You must promptly notify us of any unauthorized access or security breach at support@formlabapp.com.

We reserve the right to suspend or terminate accounts that violate these terms or that we determine, in our sole discretion, are being used fraudulently.

3. User Roles

FormLab supports two user roles, each with distinct responsibilities:

Coaches

Coaches create and manage subscription tiers, provide video feedback to athletes, and receive payments through Stripe Connect. As a coach, you are responsible for:

  • Delivering feedback in a timely and professional manner as described in your subscription tiers
  • Setting appropriate pricing and clearly communicating what athletes receive
  • Complying with Stripe’s terms of service and all applicable payment regulations
  • Ensuring your coaching advice is accurate and does not cause harm

Athletes

Athletes subscribe to coaches, submit videos for review, and receive feedback. As an athlete, you are responsible for:

  • Submitting videos that comply with acceptable use guidelines
  • Paying subscription fees on time and in full
  • Engaging respectfully with coaches through the platform

4. Content Ownership

FormLab respects your ownership of the content you create and upload.

  • Athlete videos: Videos uploaded by athletes remain the sole property of the athlete who uploaded them. FormLab does not claim ownership over athlete video submissions.
  • Coach feedback recordings: Screen recordings, audio feedback, and written comments created by coaches during their coaching sessions remain the intellectual property of the coach.
  • License to FormLab: By uploading content to the Service, you grant FormLab a non-exclusive, worldwide, royalty-free license to host, store, display, and transmit your content solely for the purpose of operating and improving the Service. This license does not allow FormLab to sell your content or use it for advertising.
  • Deletion: You may delete your content at any time through the Service. Upon deletion, FormLab will remove the content from active storage. Residual copies may remain in backups for up to 90 days.

You represent and warrant that you have all necessary rights to the content you upload and that your content does not infringe any third-party intellectual property rights.

5. Payment Terms

FormLab uses Stripe Connect to facilitate payments between athletes and coaches.

Fee structure: Coaches set their own subscription prices. FormLab charges a total platform fee of 10% on each transaction, which includes Stripe payment processing fees. Coaches receive the remaining amount after this fee is deducted.

Recurring subscriptions: All subscriptions are billed on a monthly recurring basis. By subscribing, you authorize automatic monthly charges to your payment method on file.

Cancellation: You may cancel your subscription at any time. Cancellation takes effect at the end of the current billing period. You retain access to coaching services until the period ends.

No refunds for partial periods: We do not provide refunds or credits for partial subscription periods. If you cancel mid-period, your subscription continues until the end of that period, after which no further charges will be made.

Payment disputes: Any payment disputes must be raised within 30 days of the charge. Contact us at support@formlabapp.com with details of the disputed charge.

6. Acceptable Use

When using FormLab, you agree not to:

  • Harass, threaten, intimidate, or abuse other users
  • Upload illegal content or content that violates third-party rights
  • Send spam or unsolicited communications through the platform’s messaging system
  • Attempt to reverse engineer, decompile, or extract the source code of the Service
  • Use automated bots, scrapers, or other tools to access the Service without permission
  • Impersonate other users or misrepresent your identity or affiliation
  • Upload content containing malware, viruses, or other malicious code
  • Engage in any activity that interferes with or disrupts the Service

FormLab reserves the right to suspend or terminate any account found to be in violation of these acceptable use policies, with or without prior notice depending on the severity of the violation.

7. Service Availability

FormLab is currently provided as a beta service. By using the Service, you acknowledge that:

  • The Service is provided “as is” without warranty of uninterrupted availability
  • The Service may contain bugs, errors, or other issues typical of beta software
  • We will make reasonable efforts to maintain uptime and address issues promptly
  • Scheduled maintenance may occasionally cause temporary unavailability

FormLab reserves the right to modify, suspend, or discontinue any feature or aspect of the Service at any time, with reasonable notice where possible. We are not liable for any modification, suspension, or discontinuation of the Service.

8. Limitation of Liability

To the maximum extent permitted by applicable law:

  • FormLab is not liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Service
  • FormLab is not liable for damages arising from loss of data, loss of profits, or business interruption
  • Our maximum liability to you for any claim arising from your use of the Service is limited to the total fees you paid to FormLab in the 12 months preceding the claim
  • Some jurisdictions do not allow limitations of liability, so these limitations may not apply to you

The coaching advice and feedback provided through FormLab is for informational and educational purposes. FormLab is not responsible for any physical injury or harm that may result from following coaching advice received through the platform.

9. Changes to Terms

We may update these Terms of Service from time to time to reflect changes in our practices, legal requirements, or Service features. When we make material changes:

  • We will update the “Effective date” at the top of this page
  • We will notify users via email for significant changes
  • For minor changes, posting the updated terms on this page constitutes notice

Your continued use of the Service after changes are posted constitutes your acceptance of the updated terms. If you do not agree to the updated terms, you must stop using the Service.
